1. The nominator must be a member of Ohio Good Sam – belong to a chapter or pay State Dues of $3.00 if At Large.

2. The Personal Data Sheet should be completed jointly by the nominator and nominee. The information on this sheet will only be used if the nominee becomes the recipient of the scholarship.

3. Nominee should complete the questions on the Scholarship Application Form. No reference to nominee’s name or where they reside should be mentioned. The name of the high school attended should not be stated.

4. When completed, return both forms to:

 State Director, Dave Spragg,  3930 Old Wheeling Rd, Zanesville, OH 43701

He must receive these forms prior to leaving for State Samboree, the dead line is June 24.   Allow three to five days for mail service.

5. Each application will be assigned a number that only the director will know who submitted it.

6. All applications will be turned over to the committee who will make the decision as to whom the scholarship(s) will be awarded.

7. The scholarship check will be issued to the college or institute of higher learning that the recipient will be attending in fall quarter or semester after validation of submitted information.

8. The recipient agrees that the information provided can be used in an article sent to Highways Magazine and/or local newspaper.

9. Non-winners can apply the next time a scholarship will be offered.

10. All applications will be put on file for a period of 1 year in case the named recipient fails to comply with the rules as stated or provided misleading or willful omission of information in which case another nominee will be selected.
